Referrals come from the local authority. Lyndale is able to provide support to service users who are receiving 117 Aftercare, on Section 17 leave, a supervised discharge, Guardianship order or Community Treatment Orders. Lyndale cannot support and accommodate persons who require nursing care that cannot be managed by the community district nurse service.

Before service users decide to move in to Lyndale, an assessment will be carried out by the service user and the registered manager, or manager from Lyndale and any other person the service user would like present. This assessment will form the basis of the service user plan that will be devised and formulated with the service user together with their keyworker from Lyndale and will be amended and updated to reflect the changes in the service user’s needs. The needs of the service user are assessed using our Assessment Form. This assessment is carried out by the Registered Manager or the Manager of Lyndale, the service user and sometimes with the referring Care Manager.
